NHS Test & Trace Information

It is now a legal requirement for all customers (over the age of 16 years) to provide their name and contact details when visiting a hospitality venue. These records will be kept for 21 days and provide data to NHS Test and Trace if required.

You can “check in” using the NHS COVID-19 App and scanning the NHS QR Code Poster or completing your details below.

If you use the QR Code to “check-in” the app will give you advice about what to do if you have been in close proximity to a new COVID-19 case. The app works using your Bluetooth and doesn’t share any information with us.

Alternatively, completing the table, will allow the Test & Trace team to rapidly detect people who have recently come into close contact with a new  COVID-19 case, allowing them to minimise the transition of the virus, which is crucial in protecting the NHS, Social Care sector and in saving lives.

This information will be stored securely and destroyed after 21 days. The 21 days reflects the incubation period for COVID-19 (which can be up to 14 days) and an additional 7 days to allow for testing and tracing.
